I keep personal details about you such as your contact details and issues presented in therapy in order to be as effective as possible in my work with you. I will only contact you for the purposes of arranging appointments or if we have made an arrangement for me to contact you. I will keep your details for three years after the end of your therapy in case you decide to return or you need access to your details for any other reason. You have the right to see the information I hold about you, please speak to me about it if you wish. Requests will need to be in writing. You can also ask me to change any information that I hold about you that is incorrect and/or delete information that I hold about you, save for that which I have a contractual obligation or ethical responsibility to keep.
I am committed to ensuring that your privacy is protected. Should I ask you to provide certain information by which you can be identified, then you can be assured that it will only be used in accordance with this privacy statement and will not be passed on to any third parties. Data stored digitally is password protected and all paper records are stored in locked cabinets.
